Medical paperwork is being transitioned to Athletic Trainer System (ATS) with the exception of the physical evaluation form, a copy of your insurance card, and the banned substances exemption form (if warranted). Please click on the link below for instructions to fill-out the required medical paperwork. This information must be updated every school year.

2017-2018 MEDICAL FORMS & ATS INSTRUCTIONS Physicals can be uploaded to the "e-files" section on ATS. Insurance cards can be uploaded into the insurance area on ATS. If the paper medical physical form and insurance cards cannot be uploaded into ATS, they can be mailed or handed to an athletic training staff member by the respective due dates below.

MEDICATION EXEMPTION FOR BANNED SUBSTANCES FORM To check if you are taking a medication that is banned by the NCAA, please click on this link http://www.dfsaxis.com. The organization is NCAA Division III and the password is ncaa3. IF YOUR MEDICATION IS BANNED, YOU WILL NEED THIS FORM. Give this form to the physician that prescribed your medication for them to complete. We will need the paper form on file before you can compete.
